This research aimed to demonstrate the biomechanical properties present in the diverse range of abdominal aortic aneurysm (AAA) presentations observed in real patients. The 3D geometrical attributes of the AAAs we analyzed, combined with a realistic, non-linearly elastic biomechanical model, were essential to our methodology.
Three patients with infrarenal aortic aneurysms, categorized by their clinical conditions (R – rupture, S – symptomatic, and A – asymptomatic), were subjected to a study. Steady-state computational fluid dynamics simulations, carried out in SolidWorks (Dassault Systèmes SolidWorks Corp., Waltham, Massachusetts), were employed to analyze the interplay of aneurysm morphology, wall shear stress (WSS), pressure, and flow velocities on aneurysm behavior.
Patient A and Patient R displayed a diminished pressure in the inferior, posterior region of the aneurysm compared to the rest of the aneurysm's structure, as determined through WSS evaluation. Foodborne infection The aneurysm in Patient S exhibited a remarkably uniform WSS distribution, in contrast to Patient A's localized high WSS areas. The unruptured aneurysms (subjects S and A) presented substantially elevated WSS values compared to the ruptured aneurysm of subject R. An increasing portion of the U.S. population has become reliant on hemodialysis. Issues with dialysis access represent a substantial burden of illness and death for patients experiencing end-stage renal disease. The gold standard in dialysis access procedures has been the creation of an autogenous arteriovenous fistula via surgical intervention. For those patients excluded from arteriovenous fistula creation, arteriovenous grafts, which use a spectrum of conduits, have become a widely implemented approach. We present the results of using bovine carotid artery (BCA) grafts for dialysis access at a single institution, and critically evaluate them against the results of polytetrafluoroethylene (PTFE) grafts.
Using an Institutional Review Board-approved protocol, a single-institution retrospective review was conducted encompassing all patients undergoing surgical implantation of bovine carotid artery grafts for dialysis access from 2017 to 2018. The entire cohort's patency, encompassing primary, primary-assisted, and secondary types, was evaluated, with the results stratified by gender, body mass index (BMI), and the indication for use. A comparison of PTFE grafts with grafts performed at the same institution between 2013 and 2016 was executed.
For this study, one hundred and twenty-two patients were selected. Among the patients studied, seventy-four received a BCA graft, and forty-eight received a PTFE graft. The BCA group exhibited a mean age of 597135 years; the PTFE group, conversely, displayed a mean age of 558145 years, resulting in a mean BMI of 29892 kg/m².

The investigation into BCA graft survival probability in male and female groups highlighted a statistically significant difference (P=0.042) in primary-assisted patency, with males showing better results. Secondary patency exhibited no significant difference between the sexes. Across BMI groups and treatment indications, there was no statistically substantial variation in the patency of BCA grafts, whether primary, primary-assisted, or secondary. Statistical analysis indicated an average bovine graft patency of 1788 months. Interventions were required on 61% of the BCA grafts, a notable 24% of which needed multiple interventions. Intervention, on average, was delayed by 75 months. A comparison of infection rates between the BCA and PTFE groups revealed 81% in the BCA group and 104% in the PTFE group, demonstrating no statistically significant difference.
Our study indicated higher patency rates for primary and primary-assisted procedures at 12 months, compared to the patency rates for PTFE procedures at our institution. At 12 months, the patency rate of primary-assisted BCA grafts was demonstrably greater in male patients compared to the patency rate observed in the PTFE graft group. Neither obesity nor the requirement for a BCA graft demonstrated an impact on patency rates within our observed population.
Our analysis of 12-month patency rates reveals that primary and primary-assisted procedures in our study performed better than those using PTFE at our institution. At the 12-month mark, male patients receiving BCA grafts with primary assistance exhibited a superior patency rate in comparison to those receiving PTFE grafts. Our findings suggest no correlation between obesity, BCA graft use, and graft patency in this patient group.
